Financial Policy & finance options

Smiles on Randolph strives to serve as many patients as possible in our community. Our office asks for a deposit to reserve your appointment. Any remaining balances are the responsibility of the patient and are to be paid either prior to scheduling or at the next appointment. For major dental and cosmetic services we do ask for a deposit and that the balance be paid at the time of service. We welcome all patients to our dental spa, and can answer any questions at your initial appointment.

Our team recognizes that your time is valuable, which is why we strive to accommodate you at a time that is convenient with your schedule. We are happy to schedule any appointments with you over the phone during business hours or you may use our online booking service by clicking below. We require a 48-hour notice for canceling appointments, otherwise a cancellation fee will apply.

Insurance information

Smiles on Randolph is proud to serve Crown Point, IN and the surrounding areas. We participate in many PPO insurance plans, and while this information will be verified at your initial appointment, some insurance plans we accept include Sunlife and Zelis. We are in-network with many insurance plans, and can review your insurance coverage at your initial appointment. If you are out-of-network or don’t have insurance, please call our office to discuss your options.

Our patient forms can be found below. We request that all new patients print out the forms and complete them at home. If you are unable to bring the forms completed, we request that you arrive 15-20 minutes early to fill the paperwork out in our office. Please call our office if you have any questions regarding the paperwork. By completing these forms ahead of time, you can reduce your time spent in our office, plus our team can better prepare for your upcoming visit. If you have previous records including x-rays or a treatment plan from your previous dentist, we request you bring them to your initial appointment or e-mail them to us at info@smilesonrandolph.com when scheduling.
